More than 75% of people admitted to the hospital will not be able to communicate their wishes for themselves.   

An Advance Care Plan will ensure others know the decisions you have made in the event you cannot communicate; such as life prolonging measures, organ donation and comfort care. 

I will help ensure all your end-of-life wishes are honored and carried out.  I will assist with practical matters such as in home support, coordination of care and providing comfort for you and your loved ones.  I can help create a legacy project and assist with final letters if desired.  As unique as each life is lived, so too is each death and I can adapt to your unique circumstances.

All loved ones will need help in providing you with care.  Whether help is needed for a brief time, a few hours or a few days each week.  I will be there when needed so you don't have to be alone.

As end of life approaches, I will be a caring and compassionate presence for you; whether that is by your bedside holding your hand, sitting in the room reading your favorite book or just being present.  I will be there with you and your loved ones to help ensure a dignified and peaceful end.
